I hosted a freeroll for new poker signups during a month at (the former) Pitbull Poker. I also offered a $50 bounty to the person who knocked me out. I got 6 signups, and all 7 of us were seated at the table. When the tournament was supposed to start we instead all got kicked off the table with the pop-up notice that the minimum for the freeroll was 10 players. Very embarrassing, and tough to run credible promotions for a while.

Oh man that’s a good one… personally and irrefutably embarrassed… but a totally honest mistake I would have done too!
