Welcome to CAP!

You just have to do an AddType in your .htaccess file (if you are hosted on a Linux/Unix host)

If you use PHP, its easy to do that without handler.
But both has the same value. One more thing that i use my directory links like
http://www.mysite.com/directory/
i never link to like http://www.mysite.com/directory/index.html OR http://www.mysite.com/directory/index.php
Using http://www.mysite.com/directory/ has advantage that i can anytime change extensions and page rank will not effect.

The only time you hear of issues is with dynamically built pages where a “?” needs to be used. However, our site uses these on about every page without any issues.
